Based on record review, the facility failed to protect the Private Health Information (PHI) of Patient A. This failure caused a breach of Patient A's PHI.Findings:The facility Breach Report, dated 4/1/13, was reviewed on 4/9/13. The report indicated Social Worker 1 sent a fax in error to the Public Health Department. The document contained the following PHI: parent name, address, phone number, child's name, date of birth, and diagnosis.The fax was destroyed.The facility investigated the breach, and provided training and counseling to Social Worker 1.
