Issue:
Case Management clinic notes were inadvertently faxed to the wrong vendor on July 1, 2014. The error was discovered when the wrong vendor contacted the sender for additional information. The VA Employee instructed the wrong vendor to shred the mis-faxed information. The VA Employee subsequently refaxed the information to the correct vendor.
Outcome:
07/02/14: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise, as this is a vendor that VA regularly shares sensitive personal information with.
