Issue:

Pharmacy personnel was auditing the pending prescriptions. When the check and balance was performed, it was noted that a pending prescription had been filled and mailed. The mail outs were double checked and it was discovered that one Veteran's medication was accidently addressed and mailed to another Veteran's residence. The "incorrect" Veteran agreed to mail back the incorrect prescription in an envelope provided by this pharmacy and the tablets will be destroyed upon receipt back of this envelope. The MD was contacted and reentered a new prescription for the medication to be mailed to the correct Veteran's address. Only the labeled (Veterans name, date of birth, Rx number) bottle of medication was incorrectly mailed. No other identifying papers or SSN or other identification was incorrectly mailed.

Outcome:

09/12/13: One Veteran will be sent a notification letter.
