Issue:

Today, the following incident was reported to the Privacy Officer (PO), Murfreesboro Campus: On 12/30/14, Veteran A's wife presented to the window, Outpatient Pharmacy (OP), Nashville Campus, to pick up her husband's medications. Veteran A's wife was also inadvertently given a bag of medication which belonged to Veteran B. Later in the day, when Veteran B presented to the window, OP, and his medications could not be located, they were immediately re-processed. Days later, Veteran A's wife returned Veteran B's medication to the OP, Nashville Campus.

Outcome:

01/13/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
