Issue:
On 01/06/15, Veteran A reported to the VA Pharmacy window to pick up his medications. Upon Veteran A returning home, he then realized that he was provided the medications for Veteran B. Veteran A returned the incorrect medications and Pharmacy Service has made the corrections to insure both Veterans received the correct medications.
Outcome:
01/13/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
