Issue:

Veteran A had a post discharge encounter with the provider. He stated that when he received his discharge papers on the date of discharge, 06/27/14, that he also received Veteran B's information. The provider stated that patient refused to provide him with the copy and would only provide it to the Privacy Officer (PO). The PO called Veteran A and discussed the issue. Veteran A is to mail the document to the PO. He was very reluctant to describe what the form was with Veteran B's information. He kept repeating "medical information". This Veteran is concerned that we have now disclosed his information to someone else. The PO assured him that she would investigate that possibility also, once she received the paper that he is mailing to her.

Outcome:

08/12/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services, as his full SSN and other information were disclosed. There is no evidence to show that Veteran A's information was disclosed inappropriately.
