2. The Governing Boby failed to ensure that Patient Rights were protected and enforced when a safe patient environment was not provided and patient privacy was not ensured. (See A115, A144 and A146)3. The facility failed to ensure nursing services were provided in accordance with professional standards of practice and in a well-organized manner to ensure patients' safety in the hospital.
