2. The Governing Boby failed to ensure that Patient Rights were protected and enforced when a safe patient environment was not provided and patient privacy was not ensured. (See A115, A144 and A146)3. The facility failed to ensure nursing services were provided in accordance with professional standards of practice and in a well-organized manner to ensure patients' safety in the hospital. (See A0385, A0395 and A0396) The cumulative effects of these systemic problems resulted in the Governing Body's inability to provide services and care in a safe and effective manner in accordance with the statutorily-mandated Conditions of Participation for Governing Body.
