Veteran A was given Veteran B's lab results on 07/02/13 by his provider. It was not discovered until next day. Veteran A hand-delivered the 3-page lab report to the same Community Based Outpatient Clinic (CBOC) where the error occurred. The lab report contained Veteran B's full name, full SSN, DOB and lab results.
Outcome:
