Veteran A presents with a lab order from a non-VA doctor. The Veteran is asked to wait in one room and his lab order is left in another room where Veteran B has access to the paperwork. Veteran A says that he witnessed Veteran B look at the paperwork which contains Veteran A's personally identifiable information (PII) and protected health information (PHI).

Outcome:

05/06/15: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
