Issue:
On 09/16/14, we transferred copies of electronic medical records belonging to Service Member A to a military base as requested. The personnel at the military site printed these documents on 09/30/14, and gave them to the Service Member. On 10/01/14, Service Member A called the base personnel to inform them the three pages of the records he received did not belong to him. The three pages belonging to Service Member B shows the complete name, last four of his Social Security Number, blood pressure reading and visual acuity.
Outcome:
10/23/14: The Incident Resolution Service Team has determined that Service Member B will be sent a general notification letter.
