Issue:

The Fee program received information from an outside provider regarding a Veteran. The Fee program sent a letter to the Veteran but used a wrong address. The Veteran was not registered at our facility. He was registered at four of our VISN facilities. The information went to a Veteran registered at our facility. The address was his but the name on the envelope was not. That Veteran took the letter to one of our clinics, who informed the Privacy Officer (PO). The information on the letter was the Veteran's full name, the name of the provider and the date the episode of care began.

Outcome:

09/19/13: The Veteran will receive a HIPAA letter of notification.
