Issue:
On 06/26/15 at 1:00 PM, the Privacy Officer (PO) received a call from the OR that on 06/26/15 Veteran A called and informed him that not only did he receive on 06/25/15 his own discharge instructions from same Day Surgery (SDS) but he also received Veteran B's discharge instructions. Both Veterans have the same last name. Veteran A advised that he destroyed Veteran B's discharge instruction that was accidentally given to him in SDS by an RN. The following protected health information (PHI) on Veteran B was exposed to Veteran A: Veteran B's full name, address, SSN, DOB, and medication list. The OR followed up with an email to the PO at 1:23 PM and the PO received it at 2:33 PM.
Outcome:
06/26/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
