Issue:
Four (4) completed 1010EZ's were accidentally faxed by VHA Employee to private company. Company reports shredding the documentation, but it is unclear at this time to what extent. If retrievable, will request that info is mailed back to VHA facility for proper destruction. Of note - on the EZ's, besides the info for the Veterans, there is additionally the following info: With Vet 1: Spouse name, DOB With Vet 2: Spouse name, DOB, phone number
Outcome:
05/22/14: The Incident Resolution Team has determined that four Individuals will be sent letters offering credit protection services. plus 2 spouses information was disclorsed. 06/17/14: On Veteran can not be located, as "Return to Sender, Insufficient Address, Unable to Forward." therefore only 5 people were notified.
