Issue:
A VAMC patient clinic appointment reminder letter for Patient A was mistakenly mailed to Patient B. The recipient came to the VAMC and reported the incident to a Program Support Assistant who in turn reported to the Privacy Officer (PO). Upon review of the letter, the PO noted it contains Patient A's full name and mailing address.
Outcome:
02/23/15: The Incident Resolution Service Team has determined that Veteran A will be sent a notification letter.
