Issue:

During the course of a privacy investigation, it was noted that an employee accessed the record of another employee without authorization/need to know.

Outcome:

06/18/13: This was discovered through an audit log. There was no evidence of malicious intent. 06/20/13: The employee's supervisor has initiated an investigation. 07/09/13: This incident involves a married couple who is now divorces. She is having issues involving an unknown person using her address and phone number. She asked to have a Sensitive Patient Access Report (SPAR) run . The SPAR showed that the ex-husband had accessed her record while they were still married. He was looking to see what her benefits would be once they were divorced. He has been counseled. We are paying for credit protection services on another incident. The ex-wife was given verbal notification. The DBCT requested that we obtain a Memorandum for Record that verbal authorization was given
