Issue:
Veteran A contacted the Outpatient Pharmacy, Murfreesboro Campus, to report he received a medication in the mail which belonged to Veteran B. Upon investigation, it was determined the Pharmacy, Murfreesboro Campus, incorrectly addressed the UPS label to the wrong patients name and address. Arrangements were made for Veteran A to return the package to UPS for forwarding to the medical center. Veteran Bs personal identifiable information includes his full name, address, and name of medication.
Outcome:
01/07/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
