Issue:
When the Travel Office at the VA Nashville Campus was attempting to mail a Veteran/Beneficiary Claim for Reimbursement of Travel Expenses to Veteran A, they inadvertently mailed that form along with a Travel Letter to Veteran B. Veteran Bs spouse received the mis-mailed Claim Form, as the Veteran is deceased. Veteran Bs spouse provided the mis-mailed Claim Form to her daughter, who works at the VA Chattanooga Outpatient Clinic, who then notified the Privacy Officer (PO) of this error. The personally identifiable information (PII) on the documents belonging to Veteran A included his full name, last four digits of his SSN, and home address.
Outcome:
01/06/15: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
