Issue:
On 7/28/14 at 1:51 PM, the Chief of Pharmacy sent an e-mail to the Privacy Officer (PO) about a privacy breach and attached a VHA Issue Brief. The PO didn't read Chief of Pharmacy's e-mail until 7/29/14 at 10:54 AM. Veteran B received a prescription and accompanying paperwork intended for Veteran A. The prescription was filled at the BVAMC and mailed on 7/22/14. On 7/23/14, Veteran B called to report that he had received a package (prescription and paperwork) on Veteran A. Veteran B was advised to return the package which he did and was received in Pharmacy on 7/25/14.
Outcome:
07/29/14: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
