Issue:

Last night the Education Department (ED) faxed information to a number that used to belong to a particular nursing facility. (The nursing facility had not updated a list for their own people to use and they gave the wrong number to our ED personnel when our person called.) This information ended up at Tucson Electric. A rep from there contacted us today and said they received it. They shredded the information before I was notified. I told ED the social worker who reported the incident that information needed to be returned to us so we know it was disposed of properly. Update: 07/31/12:One Patient will be sent a letter offering credit protection services due to name, full SSN, being exposed.

Outcome:

We have instituted addition staff training to ensure this type of incident does not occur in the future.
