Issue:
VA Provider or Resident left an inpatient list including first/last name and last four SSN on an ICU patient's table during morning Rounds. Within 5-10 minutes after they left the room, the patient asked a nurse what the paper was as his eyesight is very poor. The patient did not have anyone else in his room during this time.
Outcome:
07/16/14: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
