Issue:

Veteran A contacted Telecard about two prescriptions which were processed on 01/14/15 for mail that he had not received. USPS tracking revealed that both prescriptions were delivered to patient's former temporary address where Veteran lived over 10 years ago. Review of the systems reviewed change from "Not Active" to "Active" with system editing field indicating POSTMASTER AND HL FILER MONITOR/EAS ESR 675 ORU-Z05 CLIENT. It is suspected the change of Veteran A's address may been resulted from system glitch/error. The Veteran's address has been updated with current address and the medications were refilled.

Outcome:

02/04/15: The Incident Resolution Service Team has determined that Veteran A will be sent a notification letter.
