It was reported that a physician gave a patient written instructions on the back of a list that contained patients appointments. Additionally, the list contained the full name, full security social number, means test information, and co-pay information about six patients.
Outcome:
10/15/13: Six (6) Patients will be sent letters offering credit protection services. 10/24/13: Update - Seven (7) Patients will be sent letters offering credit protection services.
