Issue:

The PO received a call from the Patient advocate to come to her office. She had a patient that was upset that he received another patients information on routing slip. He said that he was given this slip by a gentleman sitting at the middle desk at check-out in Primary Care. The PO has a photo copy of the routing slip as the patient daughter has the original at home. She doesnt want to give up this routing slip till we can verify that her fathers information wasnt given out to another patient. As the PO was investigating this incident the PO was able to verify that there was no routing slip on her father. The PO has placed a call to the daughter to inform her that there was no routing slip on her father and explain how the PO was able to verify this. At this point the daughter has the original routing slip. The Patient Advocate did inform me that the daughter would mail in the routing slip once she hears back from the PO. Daughter has informed the PO that she'll be mailing the routing slip back to the Patient Advocate.
