Issue:

Employee A lost her Peer Review Committee (PRC) assignment folder. She believes she inadvertently discarded It when she cleaned out her locker last week. She had left the PRC folder in her locker for safekeeping. She did not realize it was not there until this past Saturday morning (12/06/14) when she came to work and went to look it over. She realize this is extremely serious because of the sensitivity of the information in the folder and its effect on the committees action due.

Outcome:

12/16/14: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
