Issue:
Based on interview and record review, the facility failed to prevent unauthorized disclosure of PHI (protected health information) for one patient (Patient 1) when a nurse discharged a different patient (Patient 2) with CT (computerized tomography) results for Patient 1. This failed practice resulted in the potential for financial and emotional harm to Patient 1.Findings:During an interview with the facility PO (privacy officer) on April 23, 2014, at 11:30 a.m., the PO stated Patient 2 was discharged on February 28, 2014, and when he was provided with discharge paperwork, it contained CT results for Patient 1.A review of the document given to Patient 2 indicated the document contained the following PHI for Patient 1:1. Name;2. Date of birth;3. Medical record number;4. Type of CT performed;5. Reason the CT was performed;6. Results of the CT.The information provided to Patient 2 had the potential to cause emotional and financial harm to Patient 1.
