Issue:
Based on interview and record review, the facility failed to prevent unauthorized disclosure of PHI (protected health information) for one Patient (Patient 1) when a different patient (Patient 2) was discharged from the GI (gastrointestinal) lab with his information. This failed practice resulted in the potential for financial and emotional harm to Patient 1.Findings:During an interview with the facility facility CO (compliance officer) on April 23, 2014, at 12 p.m., the CO stated two patients (Patients 1 and 2) had procedures performed in the GI lab on December 10, 2013. The CO stated when Patient 2 was discharged, she was given discharge information for Patient 1.A review of the documents given to Patient 2 indicated they contained the following PHI for Patient 1:1. Name;2. Age;3. Date of birth;4. Gender;5. Medical record number;6. Medical history (blood in stools and atrial fibrillation (abnormal heart beat);7. Name of procedure performed (Upper GI endoscopy [looking at the lining of the esophagus, the stomach, and the first part of the small intestine through a camera]);8. Reason for the procedure (blood in stools)9. Date and time of the procedure;10. Physician performing the procedure;11. Medication administered;12. Summary of the procedure; and,13. Findings (hernia, inflamed gastric and duodenal linings, and biopsies taken); and,14. Pictures taken during the procedure.
