Issue:

Veteran A, 70 year old, 100% SC Veteran presented to the Chief of Social Work's office with a list of appointments on a piece of paper printed by an unknown VA staff at 11:00 AM on 01/03/14. The Veteran stated " I have been looking for you all day. The people on the other side did not know where you were." He admitted to having some difficulty because of health with the travel around the VAMC to locate Social Worker. She reviewed the appointment list and questioned the Veteran. He informed her that he had his own place and was 100% SC. When she addressed him as Veteran B (the name on the appointment list that he gave to her), he stated, Veteran B, my name is Veteran A. Veteran A had been inadvertently given the appointment list intended for Veteran B. Veteran B is enrolled in the HCHV Program. According to the Veteran, he was given the appointment list by a staff in the diabetic clinic.

Outcome:

01/07/14: Veteran B will be sent a letter offering credit protection services.
