Issue:
Based on interview and record review, the hospital failed to ensure that Patient 1's protected health information (PHI) was kept confidential from anyone not authorized access.Findings:An interview with the Director of Quality (DOQ) was conducted on 10/1/12 at 3:15 P.M. The DOQ stated that Patient 1's preliminary radiology report was inadvertently mixed with Patient 2's radiology report and were both given to Patient 2. A review of the preliminary radiology report indicated Patient 1's name, date of birth, procedure done, and the preliminary result. The DOQ acknowledged that the staff should have checked the documents to ensure that they were correct prior to giving them to the patient.
