Based on interview and record review, the hospital failed to ensure that Patient 1's protected health information (PHI), was kept confidential from anyone not authorized access.Findings:An interview with the Director of Quality (DOQ), was conducted on 10/1/12 at 1:25 P.M. The DOQ stated that Patient 1's prescription label was mistakenly placed inside another patient's prescription bag. A review of the prescription label indicated Patient 1's name, home address, date of birth, phone number, physician's name, and medication. The DOQ stated that the pharmacy staff should have checked the prescription bag to ensure that the medication and label matched and were correct.
