Issue:
A VA employee reports that an unemployment form for eligibility from the Department of Workforce Development that contains full name, SSN, and gross wages was found in the Canteen by Canteen employee. The paperwork was returned to the employee's supervisor for action, but it is unclear how long the information was public-facing. The information was returned on Saturday, 12/20/14.
Outcome:
12/22/14: The Incident Resolution Service Team has determined that Individual A will be sent a letter offering credit protection services.
