Issue:

On 3/11/14 at 2:46 p.m. Chief of Pharmacy notified PO that a veteran received another veteran's prescription and prescription supply items (gauze and tape). On 3/11/14 at 2:51 p.m. Chief of Pharmacy supplied PO and Executive Leadership with an Issue Brief as follows: On 2/27/14 a package was mailed to Veteran A that contained a prescription and prescription supply items (gauze and tape) intended for Veteran B in error. Veterans A and B have the same last name. On 3/11/14 Veteran A spoke with a Clinical Pharmacist and reported that he had received a prescription, gauze and tap in the mail intended for Veteran B. Veteran A stated that he planned to return the items intended for Veteran B to our CBOC in Maxwelton, WV. The prescription and supply items intended for Veteran B will be dispensed again and mailed to Veteran B. The pharmacy staff member involved in packing the prescription incorrectly was counseled concerning this mistake. The Chief of Pharmacy reiterated the correct process for mailing prescriptions to patients, step by step, with the pharmacy staff member. The pharmacy staff member was reminded that it is expect that all names are double checked, without exception, before packages are mailed to Veterans.

Outcome:

03/11/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
