Issue:

A report of contact written by C&P Clerk A was received by the Alternate Privacy Officer (PO) which described the patient's ward clerk as coming to the C&P Office to ask an NVCC question. By the asking of the question, C&P Clerk B was made aware that the patient was going out by ambulance to have a procedure done. C&P Clerk B is dating the son of the Veteran. The son of the Veteran is not listed in CPRS as a next of kin nor the emergency contact. C&P Clerk B called the son and informed him of the event, asking him if he was going to contact his mother or should C&P Clerk B do it for him. C&P Clerk B also told the son/boyfriend to take his mom up to the doctor's office and wait.

Outcome:

05/19/14: The Incident Resolution Team has determined that Patient A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
