Issue:
Veteran/Employee requested a Sensitive Patient Access Report (SPAR) report on 02/23/15. Upon review, the requester identified two Dorn VHA employees that have accessed his medical records without proper reason or authority. Dates of access for employee #1 are 04/28/14 (view registration), 04/14/14 (view registration), 09/30/13 (CPRS Chart version 1), 07/10/12 (enter authorization), and 07/10/12 (enter authorization). Dates for access into Employee #2's record are 05/20/14 (change patient), 05/01/14 (CPRS Chart version 1), and 04/16/14 (CPRS Chart version 1).
Outcome:
PO update -- Fact finding from supervisor has determined access was not justified on one employee. Action has been turned over to HR/Labor Relations to work with supervisor to determine any potential administrative actions. 03/17/15: The Incident Resolution Service Team has determined that Veteran A will be sent a notification letter.
