Issue:
Today, 01/07/14, the Pharmacy Supervisor reports Veteran B received three bottles of same medication by mail, pertaining to Veteran A. Pharmacy has attempted to contact the caretaker by phone. It is not yet determined if Veteran B was an intended medication recipient for medications mailed on 12/26/13, and if so, were they received as well? Pharmacy will arrange retrieval of Rx/educational Rx materials, as well as appropriate re-issue of medications. Root cause, under investigation. Additional notification and investigation, pending.
Outcome:
01/07/14: Veteran B will receive a HIPAA letter of notification.
