Issue:

Today, 01/07/14, Pharmacy Supervisor reports privacy incident where Veteran B received diabetic test strips and protected health information (PHI) pertaining to Veteran A in the mail. Veteran B reported this to the Pharmacy/Help desk late 01/06/14. Both Veterans should have received the same diabetic test strips in the mail, varying quantities; however Veteran B failed to receive his. He erroneously believed the shipment to him was his. The documents with PHI were shredded, and then Veteran B noticed that Veteran As name was on the prescription labels (3). Pharmacy has arranged for a return mailer to be provided to Veteran B for retrieval of the test strips. Veteran B also reports that he did not receive his. Both Veterans diabetic test strips are being re-issued by Pharmacy. Additional notification and investigation, pending.

Outcome:

01/27/14: Veteran A will receive a HIPAA letter of notification.
