Issue:

A VA Employee sent an unencrypted email to a non VA employee. The email contained first, middle, and last intial along with last four of SSN. along with past appointment dates and history of narcotics use and the patients documented "Pain Agreement" in addition it specifies diagnosis of reason for visit to Emergency Department. Update: 12/11/12:The e-mail was sent to a non-VA employee who was not authorized to see the information. The Veteran involved will be sent a HIPAA notification letter.

Outcome:

Nofitication letter sent out, action taken with the employee, request ticket to be closed.
