Issue:

A VA employee was in a car accident on 12/04/12. The employee discovered a zipped binder that contained two completed Homeless Operations Management and Evaluation System (HOMES) Homeless Services Assessment Forms missing on 12/06/12. The employee has contacted the towing company and the person who picked her up from the scene of the accident but no luck in finding the forms. The forms include the patients name, full SSN, date of birth, gender and self reported medical conditions including conditions protected under U.S.C. 7332. Update: 12/11/12:Both Veterans will be sent letters offering credit protection services.
