Accessing this record would have been outside of this employee's scope of duties and his professional need to know. Update: 03/28/12:The Veteran will be sent a notification letter due to his protected health information (PHI) being access and disclosed inappropriately.
Outcome:
Provided retraining. Svc Chief to impose sanctions.
