Issue:

Medical records were requested from the Office of Research Oversight (ORO) and the Research Office staff was unable to locate the file cabinet containing the records. The cabinet was moved without the knowledge of the Principle Investigator or the Research staff during a remodeling project sometime in 2011. Update: 03/28/12:The missing cabinet contained the files on 116 patients. The information in the files included the patients' name and protected health information (PHI). The 116 patients will be sent a notification letter.04/09/12:There is an update to this ticket. There were SSNs on the records in the filing cabinet. Due to this, Credit Protection Services will be offered.07/11/12:26 of the 116 cannot be identified. Ticket updated to reflect this information.

Outcome:

Credit Monitoring lettes have been sent. Future moves of equipment to be properly documented to ensure misplacement of files doesn't recur.
