Issue:
Veteran A contacted the Bangor CBOC stating he received a letter with lab results and a diagnosis on Veteran B. Veteran A was notified to mail the letter back to the VA for proper disposal. Veteran B will be notified of this event by the PO today. Staff were notified at Bangor CBOC of event and were educated regarding eliminating potential privacy violations in the future. Update: 01/23/12:Veteran B will be sent a letter offering credit protection services, since his full SSN and other information was disclosed.
