Issue:

On 10/16/14 our Police Chief talked to the Privacy Officer (PO) at 1:00 PM (time PO got to work on 10/16) about a possible misuse that he heard about that morning. He said there is an ongoing investigation. It is possible that the Personal Identity Verification (PIV) Clerk kept a telephone number of a student that he processed into the PIV system and then later text messaged her. On 10/16/14 at 1:30 p.m. when PO got to her office there was a voice message from our Radiology Supervisor for PO to call him and there was also an e-mail from the Radiology Supervisor dated 10/15/14 at 5:27 PM stating that one of the ultrasound techs came to him on the evening of 10/15/14 stating that she had a patient that works here come by for an ultrasound. The ultrasound tech and her ultrasound student did the exam on the employee that works in PIV. Sometime after the exam had been completed the ultrasound student got a text message from the PIV Clerk. The student does not know him and believes that he got her information from where she had to do the PIV clearance. She is also concerned that he has her home information. Radiology Supervisor asked if he and ultrasound tech could come by to see PO and discuss. On 10/16/14 around 3:00 PM, the POI e-mailed Radiology Supervisor for he and ultrasound tech to feel free and stop by to discuss this matter on 10/16/14. On 10/17/14 at 8:19 AM, the PO e-mailed Radiology Supervisor that shed a meeting from 8:30 to 9:30 but should be in her office most of the day after that.

Outcome:

10/22/14: The PO reports that the PIV clerk did access the employee's record to get her cell phone number. The Incident Resolution Service Team has determined that the ultrasound student should be sent a general notification letter.
