Issue:

Veteran A and Veteran B both have the same name, but different middle names. We received a 27-0820 from the Philadelphia, NCC, which had Veteran B's SSN. We sent DTA 5103 letter to Veteran A with Veteran Bs SSN. Veteran A was contacted and asked if he could return the letter to our station and we would provide him a new DTA 5103 letter with the correct information. He stated he already sent the information back to the ROJ.

Outcome:

09/10/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
