Issue:
Veteran A sent a request for copies of his STR's to VARO 351, VARO 351 forwarded his request to the RMC. The RMC fulfilled his request including four pages of STR's for another service member by the same last name. Veteran A forwarded the four pages to VARO 531. We have identified Veteran B as being deceased as of 11/15/12. 290 facility number 290 is currently pending for widow benefits.
Outcome:
09/05/13: One Veteran will be sent a letter offering credit protection services due to full name and SSN being disclosed.
