Issue:

On 3/4/14 a prescription for the same medication and same directions for Veteran A and Veteran B were placed in the wrong mailing package, in error, before the packages were mailed. Veteran A and Veteran B have the same last name. Veteran A received the prescription intended for Veteran B and Veteran B received the prescription intended for Veteran A. On 3/6/14 Veteran A called the Pharmacy Chief to report that he had received his prescription in the mail but another Veteran's name was on the prescription label. After the Chief, Pharmacy contacted Veteran B, it was determined that Veteran B received his prescription in the mail also and it had Veteran A's name on the label. Veteran B stated he was returning the mailing package and all contents to the medical center on 3/6/14. Veteran A was instructed to mail the prescription he received for Veteran B and any accompanying documents back to the medical center in a postage paid mail we are providing. The prescriptions for Veteran A and Veteran B are being reissued.

Outcome:

03/06/14: The Incident Resolution Team has determined that Veteran A and B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
