There is an ongoing OIG investigation involving identity theft.

Outcome:

07/10/13: OIG sent the raw list of Veteran names to the Director's Administrative Officer on 07/05/13. The Privacy Officer (PO) was able to contact the OIG investigator (on 07/07/13 - 07/09/13) to confirm the information received and confirm that they could proceed with releasing the information and letters. This is an ongoing investigation involving the VA Police, OIG, County Sheriff, and IRS. The names and raw list provided (approx. 512) were researched and they were able to identify 443 individuals in their system. The data elements at risk include the individuals' name, address, date of birth, full SSN and diagnosis. The 443 individuals will receive a letter offering credit protection services. 07/15/13: Of the 443 individuals, there are 116 that VA does not have any information on. The PO checked the facility systems and the systems for all of VISN 8. 07/16/13: The DBCT reviewed this ticket and suggested that the PO send the information that he has on the 116 individuals to the Health Eligibility Center (HEC) for them to determine if the individuals are Veterans and where they are enrolled. 07/19/13: The facility PO forwarded the list of SSNs to the HEC PO. 07/30/13: The HEC PO ran a computer match on all 116 individuals with their database however they were only able to identify 21. Their review is complete. The DBCT reviewed this incident and determined that the 21 additional Veterans will receive letters offering credit protection services. VA is unable to identify the remaining individuals. No additional action is required for the remaining 95 individuals that cannot be identified. 08/14/13: The final count is 380. Those 380 Veterans will receive a letter offering credit protection services.
