Issue:
medical records sent via US Postal Service did not reach their destination as requested. Update: 01/18/11:The Veteran will be sent an offer for credit protection services, since SSN was disclosed.
Outcome:
We will resend the medical records as requested by the life insurance underwriter. We can and will now download to a CD rather than paper format. This will be sent UPS rather than US Postal Service, for tracking purposes. In reviewing this request, the facility had printed 424 pages of medical information. The veteran received only 42 pages of information back from the US Postal Service when he was notified that the package was damaged at the Kansas City Post Office. In light of the volume of unaccounted pages, I would like to request the offering of credit monitoring services for this veteran.Although this information was intact when it left the VA facility, we have no accounting from the US Postal Services - only the notification that the package was damaged en route to its destination.Please upgrade this to an incident. Veteran has since contacted the VISN office, hoping for prompt action/response.
