Issue:
A SF-182 requesting tuition reimbursement to attend a training seminar at a private sector Medical Center in San Juan PR was placed in our facility inter office messenger envelope to go from the service chief to the Chief of Staff for approval. Messenger envelope was inadvertently placed in the the US Mail Box and went to US Postal office. Post Office opened and mailed information to the medical center in San Juan PR. When Medical Center in PR determined it did not belong at their medical center they mailed it back to the Post office. Post office staff then mailed all documents, including all previous mailing envelopes to the employee who name and address was listed on the form. The SF-182 includes, employee name, full ssn, home address. Update: 01/11/11:The employee will receive a letter offering credit protection services.
Outcome:
Documents returned to employee and secured. Service Chief aware of incident and staff counseled to take additional precautions with mail sorting.
