Issue:
Based on staff interview and document review the facility failed to prevent an unauthorized disclosure of PHI for 2 of 2 patients reviewed (1, 2).Findings:1. On 7/10/14 the facility reported a breach of PHI for Patient 1 and Patient 2. Review of facility documents indicated that documents for Patient 1 and 2's were given to another patient during the discharge process.2. Medical records containing PHI for Patient 3 were inadvertently sent to an outside insurance company. On 4/30/15 the Director of Accreditation, Regulation and Licensing was interviewed. The Director confirmed the breaches occurred as described above.
