An employee inappropriately disclosed to an attorney a copy of a patient's medical record progress note. The note contained the patient's mental health diagnosis and full telephone number
Outcome:
02/03/14: Veteran A will be sent a notification letter. 02/18/14: According to the Privacy Officer. A progress note with another patient's full name was also discovered after reviewing the information that was disclosed. Fact-finding meeting was held with employee. Preparing notification letter(s). Therefore Veteran/Patient B will be sent a notification letter.
