Issue:
A Release of Information clerk prepared and mailed a CD to Veteran A, containing a copy of his medical records. There was one document on the CD that belonged to Veteran B. The document was a FAX cover sheet that contained the patient's name, DOB and destination of fax (Cardiology Clinic). Veteran A who received the CD returned it to the Privacy Officer.
Outcome:
01/17/14: Veteran B will be sent a letter offering credit protection services.
